What is a community health assessment (CHA)?
Minnesota's community health boards are required by statute to complete a community health assessment (CHA) at least every five years. The CHA helps to determine local public health priorities by collecting and analyzing data on the health status, needs and resources of a community. The findings from the CHA are used to develop strategies to improve the health and well-being of communities.
What is a community health improvement plan (CHIP)?
A community health improvement plan (CHIP) uses CHA data to identify priority issues. It guides a community health board and its community partners on work to improve the health of its population. While we continue to work with Bloomington, Edina, and Richfield communities, the development of the CHIP is a joint effort of the community health boards of Bloomington, Edina, Richfield, Minneapolis, and Hennepin County.
