The Bloomington Public Health Division, on behalf of the Public Health Alliance of Bloomington, Edina, and Richfield (PHABER), is proud to share the release of the 2024-2029 Community Health Assessment (CHA). The full report can be found here.
The CHA focuses on mental health while also including other important data to give a complete picture of our community’s health. Community members and partners played an important role in shaping this report. Their voices helped highlight what matters most to those who live and work here.
The report is organized into five key themes:
Crisis and distress
Substance use and misuse
Social isolation and connection
Prevalence of mental health conditions
Barriers to accessing mental health care
What’s Next?
Following the assessment, we will be working with community partners and members through a series of engagement opportunities to identify priority focus areas. Using the MAPP 2.0 framework, this process helps align community resources, build on current efforts, and create realistic strategies to improve health outcomes. A mid-point data review is planned for 2026–2027.

Public Health is conducting a community health assessment (CHA) focused on mental health. We've developed this survey to identify both barriers and resources. The information gathered will be used to develop strategies to improve the health and well-being of communities.
