The Bloomington Public Health Division, on behalf of the Public Health Alliance of Bloomington, Edina, and Richfield (PHABER), is proud to share the release of the 2024-2029 Community Health Assessment (CHA). The full report can be found here.
The CHA focuses on mental health while also including other important data to give a complete picture of our community’s health. Community members and partners played an important role in shaping this report. Their voices helped highlight what matters most to those who live and work here.
Key themes that emerged included:
mental health crises and distress,
substance use and misuse (including opioid use and related deaths),
social isolation and the need for connection,
the prevalence of mental health problems within the community, and
significant barriers to accessing mental health services, particularly difficulties navigating and connecting with available resources.
What’s Next?
Following the assessment, we continue to work with community partners and members to identify priority focus areas. You can participate by taking the prioritization survey below and ranking the key themes, with one as the top priority. Utilizing the MAPP 2.0 framework, this process helps align community resources, build on current efforts, and create realistic strategies to improve health outcomes. A mid-point data review is planned for 2026–2027.

Public Health is conducting a community health assessment (CHA) focused on mental health. We've developed this survey to identify both barriers and resources. The information gathered will be used to develop strategies to improve the health and well-being of communities.
